Care Coordination Program

Stephanie is a 74-year-old female that has been enrolled on the Care Coordination Program since April 2024. Stephanie lives alone in Lima.   Stephanie has been battling her health with Lung Cancer and COPD. She wears oxygen 24 hours a day. Stephanie was unable to take a shower in her home due to her shower being upstairs. She could not climb with her oxygen. She was forced to set up her living room with a hospital bed and medical equipment and only take sponge baths in her half bath downstairs.


Now that Stephanie is enrolled on the CCP program She receives supplies, Pers unit and has a stair lift .. Not only can she take a shower now, but she has her living room back with her bedroom upstairs. This has improved Stephanie’s quality of life. With the extra cost of going back and forth to Columbus for her appointments. The help she receives from the Care Coordination program  for her supplies and PERS unit helps to offset the cost.


Stephanie has sent two cards to her Care Manager and the Agency to express her gratitude. With Stephanie’s permission, the Care Manager added Stephanie’s gratitude below.


“Thank you for being there for those of us that are older. You provide many helpful services. I am very appreciative or that is I appreciate what you do because its what we need. From the Farmers Marker benefit on up to Stairlifts transportation, etc. And I don’t just take it for granted or feel anyone owes me. I feel blessed for the services I’ve received and your agency in general. And Stacy Moore is very kind and works hard to do what she can to help. I hope all your case workers are like she is.”


“Thank you so much for all the hard work you do. I have certainly received many benefits. I think I’m still in awe of my stairlift! Can’t quite still believe it’s mine and here, Lol! You seem to go that extra mile for us, and we gain so much. I hope everything is and will go well in your life because you’ve given so much to others.”
